Case Manager - 3rd Avenue

Department: Transitional Living - DHS Women's Shelter - 70950
Location: Brooklyn - Sunset Park, NY

Case Manager Transitional Living Shelter -

Location: Sunset Park, Brooklyn, NY

Hours: Full time 35 hours per week. There are 2 shifts available. Each requires working one weekend day, and 3 late afternoon/evening shifts. Specific hours to be determined.

Case Managers will work alternate working holidays.

Salary Range: $45,000 - $50,000

Overview:

Young Adult Transitional Shelter is a 39-bed transitional shelter in Sunset Park, Brooklyn for homeless young women, ages 18 to 25. The shelter empowers women to obtain permanent housing and helps them develop the independent living skills necessary for life beyond the shelter. Residents gain access to a variety of services such as independent living skills workshops, individual and group counseling, and assistance in securing permanent housing. The shelter also partners with other BCS programs to give residents the resources they need: from connecting residents with the Education Center to get their High School Equivalency Diplomas, to directing them to our Greater Heights Clubhouse to address chronic mental illness.

Caseload approximately 19 to 20 clients

Responsibilities:

  • Provide consistent, high quality program service delivery to all clients populations in accordance with funder and regulatory requirements; best practices; BCS policies and Core Values; and Code of Conduct for Custodians of People with Special Needs. Serve as a role model to all stakeholders.

  • Navigate all client services including but not limited to referrals for specialty care (i.e. medical, mental health, and substance abuse services, etc.), coordinating appointments, and/or any other processes needed to aid residents with achieving all goals identified in their Individualized Treatment Plan and increasing their level of self-efficacy

  • Complete all required assessments and service plans in a timely manner

  • Complete progress notes within 24 hours of individual sessions

  • Meet with all clients assigned to caseload a minimum of once per week and/or as instructed by Program Director

  • Provide crisis intervention services and complete all incident reports in a timely fashion

  • Provide case management coverage in the event of staff vacancies to preserve the quality of care of all residents

  • Facilitate one group per week (i.e. mental health awareness, substance abuse prevention, game night, etc.)

  • Participate in the facilitation of fire drills

  • Aid with coordinating resident move-ins and move-outs

  • Participate in recreational events in the community to increase consumer socialization skills

  • Attend all meetings, trainings, seminars, workshops, and case conferences as assigned and required

  • Complete all other tasks as assigned by Program Director

Qualifications Required:

BSW or BA in human services related field, with a minimum of two years of experience with homelessness. Position requires direct face-to face contact with residents on an ongoing basis.

  • Ability to keep track of multiple priorities, complete tasks on time, meet deadlines and keep the appropriate people informed of the status.

  • Have great communication skills (written and oral).

  • Maintain client and staff confidentiality
  • Computer literacy (Microsoft Word, Excel, Gmail, etc.)
  • Experience with CARES Collaborative Homeless Management Information System
